Terms and Conditions (Updated 03/01/2011)
GENERAL
HD After School Care provides an environment where children can feel emotionally secured and physically safe.
Our staff strives to cultivate a positive attitude towards academics and good study habits with the children to enable them to reach their full academic potential. Homework requires responsibility from the children and must be taken seriously. Children are therefore guided to complete their homework tasks independently and with pride. HD After School Care also serves as an environment from where children can attend extramural activities of their choice. They also have the opportunity to just enjoy playing which allows their parents to excel in their careers with peace of heart.
Horse Dynamics cc (hereinafter referred to as “HD After School Care”, which term shall include the owner of the property, owner of the After Care Centre and all staff)
I, the undersigned in my capacity as parent or legal guardian of the minor child hereby agree as follows:
1. I/we, the parent(s)/guardian(s) herewith give permission for the abovementioned child to participate in HD After School’s curriculum and extramural activities as well as related outings.
1.1 The abovementioned child may be transported by HD After Care as may be necessary.
1.2 The signed transport indemnity form for HD After Care’s bus and the procedure for the use thereof is attached. Transport services will be contracted out initially.
2. We accept that HD After Care (in the sole discretion of the owner and his staff) will take reasonable precautions for the safety and wellbeing of the child during his/her/their attendance of HD After Care, but that HD After Care, its owner and staff, does not accept responsibility for any damage (direct or indirect or consequent damage) which the child may incur prior to, during or after attending HD After Care and/or any activities of HD After Care.
2.1 I/we accept responsibility for any medical or hospital expenses in case of an injury to the child.
2.2 I/we indemnify HD After Care, its owner, staff and authorised persons of any liability of any nature.
3. I/we carry my/our liability as parent(s)/guardian(s) over to HD After Care or his representative for any medical/surgical treatment that the child may need.
3.1 To my/our knowledge the child is physically able to participate in any activities and he/she is in good health.
3.2 I/we will inform HD After Care and its staff in advance in writing of any special arrangements with respect to the child and/or the child’s special needs and/or the child’s participation in activities.
4. Collection of the child/children will entirely be at own risk.
4.1 Should I/we not be able to personally collect my/our own child from HD After Care, I/we will confirm with the After Care Principal in writing the details (name, surname and contact details) of the person who will collect my/our child.
5. I/we further declare that:
5.1 Ì/we have fully completed this enrolment form and that the details contained herein are true, just and correct.
5.2 I/we will inform HD After Care immediately of any change in my/our contact details.
5.3 I/we are aware of HD After Care’s rules, a copy of which I/we have received.
6. I/we undertake to pay the agreed after school care fees for the child monthly in advance, punctually, failing which HD After Care or its representative may take the necessary legal steps to recover the outstanding fees, in which case –
6.1 I/we shall pay interest on the outstanding amount at 18.5% per annum, calculated from the date of default payment to the date of full
recovery of the outstanding amount;
6.2 I/we will be responsible for all legal costs (on attorney and client scale), interest and collection commission.
7. Despite the fact that HD After Care requires both parents/guardians (if applicable), to sign this document, my signature will be accepted as sufficient to bind me to the conditions contained herein. Should both of us sign this document, we will jointly and severally be responsible for all obligations contained herein.
8. Any changes hereto will be in writing to be legally valid.
PAYMENT SCHEDULE 2012:
OPTION 1:
Annual Administration fee (once-off): R150.00 (Includes a FREE Horse Dynamics bandana - while stocks last)
Quarterly Fees: R 2 250.00 per school quarter; or
R 750.00 per month, tendered only by EFT payment
Note: 5% family discount applicable to 2nd and 3rd child
1st Term : During 18 January – 30 March 2012 (Last day of payment 7 January 2012)
2nd Term : During 2 April – 29 June 2012 (Last day of payment 7 April 2012)
3rd Term : During 2 July – 28 September 2012 (Last day of payment 7 July 2012)
4th Term : During 1 October – 7 December 2012 (Last day of payment 7 October 2012)
Fees, paid by EFT, are to be paid in advance within 7 (seven) days of the month of enrolling your child and subsequently before the 7th (seventh) of the month of each new Gauteng public school term. We are enforcing a strict “payment in advance” policy to all EFT clients. HD After Care reserves the right to charge interest on overdue accounts and/or cease offering after school care to the client. Please refer to the debit order instruction form for an easy hassle-free monthly payment option.

OPTION 2:
DEBIT ORDER AUTHORISATION
The quarterly fee will be proportionally collected monthly. If your child attends HD After School Care during the 4th quarter, the final debit order will be collected on the 24th of December 2012.

Monthly fee by automated debit order: R750.00 per child (5% family discount applicable for 2nd and 3rd child)
I/we hereby request and authorise you to draw against my/our account with the abovementioned bank (or any other bank or branch to which I/we may transfer my/our account) the sum of R750.00 (seven hundred and fifty rand). Debit orders are collected on the 27th of each month or on the day prior to the 27th in the event where the 27th falls on a Saturday, Sunday or public holiday. This being the amount necessary for settlement of the monthly fee in respect of our enrolment and indemnity agreement.
All such withdrawals from my/our account by you shall be treated as though they had been signed by me/us personally. I/we the undersigned, "instruct" and authorise your agent Netcash (Pty) Ltd, to draw against my/our account. I/we understand that if bank details have been supplied, the withdrawals authorised here will be processed by BankServ. I/we also understand that details of each withdrawal will be printed on my/our statement.
I/we agree to pay any banking charges relating to this debit order instruction.
The debit order will automatically be cancelled on the 24th of December 2012 if no cancellation has been received. Please refer to the cancellations clause of this agreement.
CANCELLATIONS
To cancel this agreement before the 24th of December 2012 and discontinue after school care, one full calendar month’s written notice from the first day of the month is required. The parent/guardian of the child undertakes not to give notice of withdrawal during the month of November or December (regardless of whether the child is returning the following year). The final date on which notice of withdrawal can take place is the 1st of October for the end of October 2012.
